Require T&C To Complete Provisioning (T&C Enrollment Step)

      Currently, T&C agreements can only be included in a self-service enrollment. By moving T&C to a separate step (before finalize but after identifiers are assigned) agreements could be required before provisioning, even for conscription/administrator style enrollments. Presumably this would generate an email along the lines of "click here to complete your enrollment".
