A CO Person registered in CO-1 executes a Self Signup Enrollment (CO-2) with an EnvSource Enrollment Source attached. The EnvSource identifier is already registered in CO-1, and the CO Person is authenticated as it.

The CO-2 Self Signup Enrollment correctly creates a new OrgIdentity from EnvSource, but then links it to the CO Person record in CO-1.
