Implement name based identifier assignment.

Support specification of type of name to pull from cm_names (official, preferred, CO, Org).

Support various combinations of first/middle/last names, first/middle/last initials, and different separators (./-/_). Perhaps allow this to be specified via a modified sprintf style format.

Support collision resolutions: Add collision #; Add middle initial, then collision #; Add middle name, then collision #

If an identifier of the target type already exists for the person within the CO, do not create a new identifier.
