If an enrollment flow is defined with an Organizational Name and "copy to CO Person record" is checked, and if no other CO Person attributes are defined in the enrollment flow, then the enrollment flow will throw an error on createPetition() since $requestData['EnrolleeCoPerson'] is empty. Probably the copy of the name is happening too late in the sequence. Note also CO-333 may be somewhat relevant.
