Index view actions and menu improvements

      The actions for an index row should be revealed in a drop down when needed, and UX best practices should be reviewed.

      Considerations:

      • Consider the use of delete on index page vs. delete only in bulk actions and on edit page.
      • When only edit or view are available, don't use an actions menu - simply link the row.
      • Consider the actions menu placement.
